THE OFFICE CONTINUES TO BE CLOSED, BUT RECORDS ARE AVAILABLE!

 

All the medical charts for patients that were active when the office closed the end of May 2011 were held at another practice until the last week of August 2014. The remaining ones are still available, but only by contacting Dr Fredenburg directly at fredenburg.david@gmail.com. Please understand that records will be made available when appropriate, but urgent requests for records now 3 years after the office closure will not be possible. When you contact Dr Fredenburg at the above email please have a scanned copied of the record request attached to your email. The information on the record request is needed to process the records correctly. A copy of form is available to be downloaded from this site as shown below or from the office you will be using for care.

 

If you have not provided a medical release form previously, please complete the form (medical record request) and include it in the email to Dr Fredenburg. (If you check any restrictions on the information to be released on the form, the chart will be reviewed relative to your specific request which will add time to the processing.) Be sure that authorizations as well as the person picking up the chart are appropriate for the age(s) of the patient(s) as well.

 

Once the records are ready you will be notified by email how to retrieve them. Please remember that charts will NOT be mailed or faxed, they will only be given to the patient or authorized parent/guardian. Also consider providing your new office with a copy you make of the original records as most offices will not give the originals back to you if you transfer care again (including the old records we have offered).

 

If you have been an inactive patient and need records, also please use the email address fredenburg.david@gmail.com to request them. A copying charge may apply for inactive patients, or if you are requesting additional copies of previously provided records.

Monday, August 15 from 8-9 PM, will be the last pickup at the 49 Derry Road office. Charts that have not been picked up or for which no chart release has been received will be moved to another location this coming week. Information on how to retrieve the remaining charts will be posted in the next week. Again, NO CHARTS WILL BE MAILED TO PATIENTS OR OTHER OFFICES. Parents/patients will need to collect the charts in person to avoid mishandling by other offices or mail, also to allow the complete record, including old records from other offices, to be available to the patient for future use.

 

Records that are ready for pickup can be seen by opening the following .pdf file, Scanned Records. This file will be updated as new charts are ready (currently charts without medical record requests are being processed but will require a signed request to be handed out.)

 

The medical record request can be downloaded as before by opening the following .pdf file (medical record request) and either email the completed form to dr.freds@hotmail.com or mailing to the office address. There is no fax line at the office at this time.

 

Please also consider settling any outstanding accounts with the office soon. Accounts with balances and no response to recent billing statements will be referred to a collection service in the next month.

During August we wil be attempting to dismantle the office materials and move them out. As a result the dates and times for pickups will be less frequent. We request you attempt to pick up requested records SOON, as future arrangements will become more difficult. Again dates and times are listed below as we schedule them.

As of Wednesday June 29th pickup time, all the charts for which we currently have release forms will be available for pickup. Please check the pdf by tomorrow evening to see if the records you are looking for are listed. If you have sent in a request and do not see them on the list tomorrow evening, please email me at the address below.

 

Also, as of Friday July 1, the fax line for the office is be discontinued. You may correspond by mail or email regarding records or other concerns.

This office closed for patient care at the end of business May 31st, 2011. Patients and insurance companies were notified by mail in April regarding the planned closure.

 

If you or your child have a health issue that needs attention, please contact your new clinician's office for care. If you have not established a relationship with a new clinic, please consider using a local emergency room or Southern NH Medical Centers 'Immediate Care'. Websites for the hospital employed or affiliated clinicians in the Nashua area are listed in the notice below. Please understand that clinical or health related questions may not be addressed through this website or email. If you have not already done so, information on requesting medical records can also be found below.

 

Patient records that are available for pick-up can be checked by opening the following .pdf file Scanned MR (an updated .pdf file will be posted as new records are available). We are processing well over 1200 records, then giving you the originals for your future healthcare needs. The records are generally being completed in the order of the request dates. If you need specific information from the chart to get established in a new office, please email the office with what chart information is required (usually most offices will allow initial visits in their offices if the immunization record and last physical are available). Medical record transfer forms are available by opening the following .pdf file (medical record request) then the completed form can be either brought in, mailed or faxed to the office. Medical records may only be picked up at the office as the chance of the records being misplaced when sent through the mail can be a problem.The email address to reach the office is dr.freds@hotmail.com.

 

The office will be open at the times listed below for medical record pick-up. Please recheck the time on this website page the day you plan to come in to be sure the time has not changed. Dear Patient/Parent,

 

We regret to inform you that this office will close at the end of May. Private practice in pediatrics in the Nashua area is no longer a reasonable option, and this has become much more apparent since the beginning of this year. The insurance companies were notified at the beginning of April and I understand that some have started to send out letters to patients. I was arranging to send out notices 2 weeks ago but one of the large healthcare systems in the area asked to acquire the practice. Subsequently they decided such a plan was not ultimately to their benefit--notifying us April 18th.

 

We are contacting patients with existing appointments after June 1st in an attempt to rebook them if possible. Please contact the office if an appointment has not been rescheduled.

 

There is an abundance of pediatricians and family practitioners in the Nashua area (in fact I am not aware of any pediatric practices that are currently closed to new patients). You may find information about all the available providers through their websites (www.stjosephhospital.com, www.snhmc.org, www.dartmouth-hitchcock.org/nashua, www.catholicmedicalcenter.org, www.elliothospital.org) We will make what efforts we can to make charts available for you to take to the new clinician (please understand that we will be requiring the parents or patients to physically pick up the chart to avoid mishandling, no charts will be sent in the mail). Please be patient as we will have a good many records to process. Medical record transfer forms are available by opening the following .pdf file (medical record request) then the completed form can be either brought in or faxed to the office. Information regarding remaining charts after the office closes or other questions will be addressed on the website (contact email regarding questions is dr.freds@hotmail.com). Be aware that clinical questions and advise are not available by email.

Patient Oriented, Physician Directed Pediatric and Adolescent Healthcare

David Fredenburg MD PA is a private, physician owned pediatric practice located on Derry Road (Route 102) in Hudson, New Hampshire providing care for infants, children, adolescents and young adults through college years.  We are the only private pediatric office in Nashua area affiliated with the local hospitals.  Health care is provided by a board certified pediatrician, a nurse practitioner, undeniably the nicest nurse in the Nashua pediatric community, and an office staff whose goal is to guide you through the world of medicine and health insurance with a minimum of frustration.

 

So in this day and age, why a physician directed, privately owned pediatric office in the Nashua area? 

In the mid-1990s, large corporations such as hospitals started purchasing most of the pediatric practices. We felt it was time to reintroduce a private pediatric office in the Nashua area.  The large hospital and insurance owned groups in Nashua where I had practiced until then felt increasingly less personal, and clearly less patient oriented.  The clinics' management seemed to be more invested in getting market share than employing a competent, consistent and compatible group of physicians offering thoughtful, rational and considerate medical care.  In short, patient care became a secondary interest for the folks at the helm.  The opportunity to open a small office with one physician and a nurse practitioner in Hudson became a reality in July 1997. In fact this pediatric practice now has the longest history in the Nashua area without a change in ownership or management.  So, with our ongoing partnership with patients and their families (with whom we weathered more than the last decade of medical, insurance and marketing changes), we continue to offer personal health care to children and adolescents.
